0165(3)(a) During the visit on 4/4/24 it was observed that the pricker bushes in the yard had not been trimmed back. 0165(4)(c) During the visit on 4/4/24 it was observed that the light bulbs in the bathroom are not shatter-resistant. 0165(5)(b) During the visit on 4/4/24 it was observed that two outlets in the kitchen near the sink were neither GFCI nor tamper resistant. Staff member T.R. found a blank switch plate on Amazon during the visit to purchase for one of them and will replace the other with a GFCI and TR outlet.
